Choose your track, review the full terms, and download your signed PDF. Then manually attach it to an email to chef@plateprep.com. This page does not send, store, or countersign your agreement, and does not complete partner onboarding.
Restaurant brokers can instead complete broker signup and sign the short agreement online to save an application, choose a payout preference, and receive a broker ID. Applications remain pending Company review and written acceptance.

2. Agreement terms
Broker introduction standard: The broker makes both a three-way email introduction and a three-way text-message introduction, with the owner's permission. A live telephone handoff is encouraged when practical. Submitting a form, name or contact record alone does not automatically create an approved or commission-eligible referral. PlatePrep separately confirms eligibility and attribution in writing, then schedules and conducts the demonstration. Help secure the demo, provide reasonable relationship follow-up when requested if an interested owner becomes difficult to reach, and check in 30–60 days after go-live. Process and copyable templates.
